Minimal-invasive Knochenentnahme aus dem Beckenkamm Minimal Invasive Bone Extraction out of the Pelvic Crest entwickelt in Zusammenarbeit mit: developed in cooperation with: 2 Knochentransplantationen im Mund-, Kiefer-, Gesichtsbereich oder in der dentalen Implantogie zählen heute zu den häufigen Operationen. Die Gewinnung des Knochen-transplantates sollte einerseits so wenig belastend wie möglich sein, anderseits eine ausreichend große Transplantatmenge liefern. Aus anatomischen Gründen sind die Möglichkeiten der Gewinnung größerer Knochentransplantatmengen im Kieferbereich begrenzt. Die Entnahme von Beckenkammknochen ermöglicht dagegen fast immer die Bereitstellung einer befriedigenden Transplantatmenge. Dieses Verfahren war jedoch bisher wegen der erhöhten postoperativen Patientenmorbidität weniger beliebt. In Zusammenarbeit mit der Firma Medicon wurde deshalb ein spezielles Instrumentarium für die minimal-invasive, kontrollierte Beckenkammknochenentnahme entwickelt. Das Instrumentarium bietet die Möglichkeit, über eine kleine operative Eröffnung einen kontrollierten Zugang zum Beckenkammknochen herzustellen. Die mediale, laterale, ventrale und dorsale Begrenzung des Beckenknochens wird dabei nicht verletzt. Durch die vollständige periostale Abdeckung des somit allseits knöchern begrenzten Entnahme-defektes ist eine ungestörte knöcherne Regeneration möglich. Knochenentnahmen an gleicher Stelle sind später erneut möglich. Ein spezielles Design der Instrumente zur Transplantatentnahme und der Knochentrepane verhindern die versehentliche Perforation der medialen Beckenwand zur Bauchhöhle. Klinische Erfahrungen zeigen, dass die postoperativen Beschwerden der Patienten sehr gering sind. Das Instrumentenset der Firma Medicon enthält alle wesentlichen Instru-mente für eine Patienten schonende Vorgehensweise und erlaubt dem Operateur die Gewinnung einer stets ausreichenden kortikalen oder spongiösen Knochenmenge. Bone transplantations in the maxillofacial region or for dental implantology are one of the most common surgical procedures. The harvesting of the bone material for grafting should be as atraumatic as possible, but it should also provide an adequate volume of bone. For anatomical reasons, the options for harvesting reasonably large volumes of bone in the jaw region are limited. In contrast, the extraction of bone from the pelvic crest almost always provides a sufficient volume of bone for grafting. However, this procedure has not been very popular in the past because of the increased postoperative morbidity among patients. A special set of instruments for minimally invasive, controlled extraction of bone from the pelvic crest has therefore been developed together with the Medicon company. The instrument set is used to establish controlled access to the pelvic crest bone through a small surgical incision. This procedure does not injure the medial, lateral, ventral and dorsal margins of the pelvic bone. The complete periosteal covering of the bone defect, which is bounded by bone on all sides, allows undisturbed regeneration of the bone. Bone can be extracted from the same location again at a later time. The special design of the instruments for bone extraction and bone trephination prevents accidental perforation of the medial pelvic wall to the abdominal cavity. Clinical exper-ience demonstrates that patients experience little postoperative pain. The Medicon instrument set contains all important instruments for an atraumatic procedure, and allows the surgeon to extract an adequate volume of cortical or cancellous bone at any time.
