Birte Melsen, Århus I ** Prof. Dr. Dr. Wolfgang Zemann, Graz Clinical Advice: • After implementation of the segment osteotomy any bony interferences on the segment and/or alveolor ridge must be smoothed before the segment distractor is installed.* • When positioning the drive module avoid damage of dental roots during fixation of the plate. In case of collision the drive module must be repositioned and refixated.* • At least 2 screws or more are needed for a stabilised fixation.* • An injured dental root is regenerating itself by creating cellular cement. (see image below)* • After wound-closure the distraction device is located orally between attached and mobile gingiva.* • When removing the segment distractor, an osteoplasty may necessary to close the gap between the segments and/or the nasal floor. Bone can be harvested intraorally from the zygomatic process of the maxilla.* • In the case of bilateral cleft palate, a two-stage treatment is recommended.**
