2 Einführung Introduction Introducción Introduction Introduzione Die Möglichkeit, Epithesen und Hörhilfen mit Titanimplantaten direkt am Knochen zu verankern, hat die Rehabilitation von Patienten mit kraniofazialen Defekten durch angeborene Fehlbildungen, Traumen oder Tumoroperationen revolutioniert. Mit keiner anderen Methode kann ein derart guter Halt der Epithesen oder Hörhilfen erzielt werden. Der behandelnde Arzt kann den Patienten in der operativen Tumortherapie ein zuverlässiges und ästhetisch anspruchsvolles Konzept zur Rehabilitation anbieten. Das ist besonders dann wichtig, wenn die Tumoroperation einen entstellenden Eingriff mit Entfernung der gesamten äußeren Nase, des Ohres oder der Orbita notwendig macht. Die implantologische Versorgung mittels Titanimplantaten und die sich daran anschließende epithetische Versorgung kann als ein Segen für die Patienten mit kraniofazialen Defekten bezeichnet werden. Das MEDICON “Ti-Epiplating System” verteilt die auf den Aufbau einwirkenden Kräfte zunächst auf die Platte und dann auf mehrere Knochenschrauben. Dies minimiert den Nachteil von Systemen mit einzeln stehenden Implantaten, bei denen alle Kräfte auf die Verankerung punktuell einwirken und stellt einen besonderen Vorteil bei geringem oder schlechtem Knochenangebot im Bereich der Orbita, der Nase aber auch in einem gut pneumatisierten Warzenfortsatz dar. Die Aufnahme der Attachments ist entfernt gelegen von den osseointegrierten Knochenschrauben, d.h. der Hautdurchtritt der epithetischen Halteelemente liegt nicht im Bereich der Knochenschrauben. Die “Ti-Epiplating System” Implantate sind aus biokompatiblen Titanlegierungen gefertigt, und die Formgebung der einzelnen Platten ist auf die Anatomie der jeweiligen Gesichtschädelbereiche abgestimmt. Rehabilitation of patients with craniofacial defects caused by innate malformation, trauma or tumour surgery, was revolutionized by the method of direct bone anchorage of craniofacial prosthesis (epithesis) and bone anchored hearing systems. By no other means, such a good retention of the prosthesis could be achieved. Thereby, the physician is able to offer a reliable and cosmetically sophisticated concept for rehabilitation after surgical tumour therapy. This is especially important, when surgery means an ablative procedure with complete resection of the nose, the ear or the orbita. Osseo-integrated bone anchorage by titanium implants and the following prosthetic care is of great benefit for the patients with craniofacial defects. The MEDICON “Ti-Epiplating System” spreads the forces acting on the superstructure at first onto the plates, thereafter onto the multiple bone screws. This minimises the inconvenience of solitary implant systems, where all forces impact lies on the fixture. Furthermore, it represents a special advantage in patients with scarce or low quality bony bed, as it is often the case in the orbital and nasal region, but also in the very well pneumatized mastoid process. The attachments are established remote from the site of the osseointegrated bone screws, i.e. the skin penetration of the attachments is not near to the bone screws. The “Ti-Epiplating System” implants are made of biocompatible titanium alloys and the design of the individual plates is adjusted to the anatomy of the different parts of the skull. La rehabilitación de pacientes con defectos craneofaciales causados por malformaciones congénitas, traumas o operaciones tumorales ha sido revolucionada por la fijación directa al hueso de epítesis craneofaciales y de prótesis acústicas contra la sordera. No existe método alguno que ofrezca una retención más efectiva de las epítesis craneofaciales y de audifonos. El médico puede ofrecer al paciente un concepto de gran fiabilidad y sofisticada cosmética para la rehabilitación después de la terapia tumoral quirúrgica. Este concepto es de especial importancia en casos en que la intervención quirúrgica tumoral significa la ablación completa de la naríz, oreja o órbita. El anclaje óseointegrado mediante implantes de titanio y el tratamiento epitético subsiguiente constituyen realmente un gran beneficio para pacientes con defectos craneofaciales. El Sistema “MEDICON Ti-Epiplating” transmite las fuerzas que actúan en la superestructura distribuyéndolas primeramente hacia la placa y luego hacia los múltiples tornillos óseos. Esto minimiza la inconveniencia de los sistemas de implantes únicos en los cuales las fuerzas actúan puntualmente sobre el anclaje. Además, el sistema constituye una especial ventaja para pacientes que disponen de una materia ósea escasa o de mala calidad, caso frecuente en la región orbital o nasal e incluso también en una apófisis mastoides muy bien neumatizada. El anclaje se encuentra alejado de los tornillos óseointegrados de fijación al hueso; es decir, la región de penetración cutánea de los elementos de retención epitéticos no se encuentra en la zona de los tornillos de fijación al hueso. Los implantes del “Ti-Epiplating System” están hechos aleaciones de titanio biocompatibles y el diseño de cada una de las placas está adaptado a la anatomía de las diferentes partes del esqueleto facial.
3 La réhabilitation des patients présentant des défauts cranio-faciaux causés par des malformations congénitales, des traumatismes, ou des interventions de chirurgie oncologique a été révolutionnée par la méthode d’ancrage osseux direct des prothèses cranio-faciales (épithèses) et d’appareils acoustiques à ancrage osseux. Il n’existe aucun autre moyen permettant d’obtenir une fixation aussi efficace de la prothèse. Le chirurgien est donc en mesure d’offrir une méthode à la fois fiable et esthétiquement satisfaisante de réhabilitation après une intervention chirurgicale oncologique. Ceci est particulièrement important si l’intervention entraîne une procédure d’ablation avec résection complète du nez, de l’oreille ou de l’orbite. L’ancrage ostéo-intégré obtenu avec des implants en titane et ultérieurement l´épithèse sont très utiles pour les patients présentant des défauts craniofaciaux. Le système MEDICON “Ti-Epiplating System” distribue les forces agissant sur la superstructure avant tout sur les plaques puis sur les vis osseuses multiples, ce qui permet de minimiser les inconvénients des systèmes d’implants isolés, dans lesquels toutes les forces agissent sur l’ancrage. Le système représente en outre un avantage particulier non seulement pour les patients chez lesquels le tissu osseux de la région orbitale et nasale est réduit ou de mauvaise qualité, mais aussi dans un processus mastoïdien largement pneumatisé. Les attachements sont éloignés du site des vis osteo-intégrées, autrement dit le point où les éléments de fixation traversent la peau ne se trouve pas à proximité des vis osseuses. Les installations “Ti-Epiplating System” sont en alliages de titane biocompatibles et le dessin des différentes plaques est harmonisé avec l’anatomie des différentes parties du crâne. La riabilitazione dei pazienti con difetti craniofacciali causati da malformazioni congenite, traumi o chirurgia oncologica è stata rivoluzionata dal metodo di ancoraggio osseo diretto delle protesi craniofacciali (epitesi) e di apparecchiature acustiche. Non essiste nessun altro mezzo che garantisce un fissaggio più sicuro delle epitesi e degli apparecchi acustici. Il chirurgo è quindi in grado di offrire un metodo al tempo stesso affidabile ed esteticamente sofisticato di riabilitazione dopo un intervento chirurgico oncologico. L’alternativa riveste una speciale importanza qualora l’intervento comporti una procedura ablativa con resezione completa del naso, dell’orecchio o dell’orbita. L’ancoraggio osteointegrato ottenuto con impianti in titanio e il trattamento epitetico successivo sono veramente di grande aiuto per i pazienti con difetti craniofacciali. Il sistema MEDICON “Ti-Epiplating System” distribuisce le forze che agiscono sulla sovrastruttura innanzitutto sulle placche e successivamente sulle varie viti ossee, riducendo al minimo gli inconvenienti dei sistemi implantari isolati, nei quali tutte le forze agiscono sull’ancoraggio. Il sistema rappresenta inoltre un vantaggio speciale non solo per i pazienti in cui il tessuto osseo della regione orbitale e nasale è ridotto o di cattiva qualità, ma anche in un processo mastoideo ampiamente pneumatizzato. Gli attachment sono lontani dal sito delle viti osteointegrate, cioè il punto in cui gli elementi di fissaggio attraversano la pelle non si trova in prossimità delle viti ossee. Gli impianti “Ti-Epiplating System” sono leghe in titanio biocompatibili e il disegno delle singole placche è armonizzato con l’anatomia delle varie parti del cranio.
